Our Safety Steps for Visiting the Practice

To ensure the safety of our staff and patients, Stow-on-the-Wold Dental Practice will be continuing to maintain the following steps, regardless of individuals’ Covid vaccination status.

Before your appointment

24-48 hours before your appointment, you’ll receive an email and/or SMS asking you to complete your ESSENTIAL FORMS either online, or by attending the practice 10-mintues early. This will include completing/updating your Medical History Form* and a Pre-Appointment Questionnaire for every visit. (* An update is legally required every 6 months.)

Please cancel/reschedule your appointment if you or anyone in your household is experiencing any symptoms and/or has been asked to self-isolate.
